Introduction: The health sector is one of the most important sectors of the country, with its policies impacting the health of the entire population. Identifying the capacities of the health sector is a critical step in making informed decisions to achieve the goals of the health system.
Materials and Methods: The current study is a descriptive-analytical and applied research conducted using a scoping review. Arsky and O'Malley's six-step protocol was used for the review. The study population included upstream documents, dissertations, theses, and accessible domestic and international articles. Electronic databases such as Web of Science, Scopus, Magiran, PubMed, Irandoc, IranMedex, SID.ir, Ensani.ir, and ISC.gov.ir were selected to search for resources. These databases were systematically searched from 2006 to December 2023 using appropriate keywords. Additionally, manual searches of references from retrieved articles were also conducted. Data analysis was perfomed with the method of content analysis. The data were coded, themed, and classified through systematic processes in this method. Data were analyzed using MAXQDA software version 2020.
Results: During the search period, 11 upstream documents and 53 studies met the inclusion criteria, which examined the capacities, challenges, and solutions in the health sector. The identified capacities included strategic documents, significant improvements in health indicators, scientific capabilities such as a rapid growth in scientific output, a strong medical education system, technical expertise in developing new vaccines, stem cell simulation, and drug production. The major highlighted challenges were related to health governance, financial sustainability, equity, conflicts of interest, health tourism, intersectoral collaboration, health system resilience, and social determinants of health.
Conclusion: Iran's health system is facing serious challenges. However, our country has considerable potential and actual capacities in this area. Optimal utilization of these capacities and leveraging past successful experiences can significantly enhance health indicators. In this regard, focusing on key aspects such as effective governance, sustainable financing, achieving equity in health, efficient conflict of interest management, and strengthening intersectoral collaborations can pave the way for profound and sustainable transformations in the country's health system.
