fa مقایسه هزینه‌های اعمال جراحی گلوبال با تعرفه‌های مصوب عمومی غیردولتی Comparison of Global Surgical Costs with Non-Governmental Approved Tariffs مقالات کامل full articles پژوهشي Research مقدمه: با توجه به اهمیت تعرفه‌ها و هزینه های اعمال جراحی گلوبال، هدف تحقیق حاضر، مقایسه‌ی هزینه‌های اعمال جراحی گلوبال انجام شده با تعرفه های مصوب عمومی غیر دولتی در یکی از بیمارستان‌های منتخب شهر تهران است. نتایج این مطالعه می‌تواند مورد استفاده‌ی مدیریت بیمارستان جهت کنترل هزینه‌های بیمارستانی و نیز سازمان‌های بیمه‌گر قرار بگیرد. مواد و رو‌ش‌ها: این مطالعه گذشته‌نگر و از نوع توصیفی-تحلیلی است که به صورت مقطعی در یکی از بیمارستان‌های منتخب شهر تهران در سال 1400 انجام شد. این مطالعه به بررسی وضعیت محاسبه گلوبال هزینه اعمال جراحی ( 864 عمل جراحی گلوبال انجام شده در یکی از بیمارستان‌های جنرال منتخب شهر تهران در سال 1400) در مقایسه با  محاسبه غیر گلوبال این اعمال جراحی می پردازد. جهت تجزیه و تحلیل داده‌ها از نرم افزار اکسل 2016 استفاده شد. یافته‌ها: این پژوهش از مجموع 100 عمل جراحی مصوب شده در نظام گلوبال سال 1400 ، تعداد 16 عمل پر تواتر در یکی از بیمارستان‌های جنرال منتخب شهر تهران را پوشش داده است. بر اساس یافته ها با در نظر گرفتن متوسط هزینه انجام شده در هر یک از اعمال جراحی گلوبال، در تمامی اعمال جراحی گلوبال بررسی شده، درآمد گلوبال کمتر از میانگین هزینه انجام شده بود. بیشترین زیان دهی هم از نظر درصد و هم از نظر ریالی مربوط به عمل)ترمیم فتق شکمی یا انسیزیونال اولیه) با میانگین اختلاف 19/22 درصد و میانگین 76162000 ریال به ازای هر عمل بود. بیشترین سوددهی هم از نظر درصد و هم از نظر ریالی مربوط به عمل (دیلاتاسیون و کورتاژ تشخیصی یا درمانی ) با میانگین اختلاف 8/96 درصد و میانگین 1467000 ریال به ازای هر عمل  بود. نتیجه گیری: نتایج حاصل از پژوهش نشان داد که تعرفه ی اعمال جراحی گلوبال، واقعی نیست و به دلیل ضعف های موجود در نحوه محاسبه‌ی آن، منجر به زیان بیمارستان‌ها می‌گردد. بنابراین برای جلوگیری از زیان بیمارستان‌ها و به حداقل رساندن ضعف های مربوط به نحوه محاسبه‌ی تعرفه‌ی اعمال جراحی گلوبال پیشنهادهایی نظیر بازبینی تعرفه‌های گلوبال اعلام شده فعلی، در نظر گرفتن عواملی چون سن بیمار، وجود یا نبود بیماری همراه، شدت بیماری، مدت اقامت بیمار و نحوه‌ی ترخیص در محاسبه و قیمت گذاری تعرفه های اعمال جراحی گلوبال و در نهایت قیمت گذاری تعرفه‌های اعمال جراحی گلوبال در هر سال متناسب با نرخ تورم واقعی کشور، ارائه می گردد. Introduction: Considering the importance of tariffs and costs associated with global surgical procedures, the aim of the present study is to compare the costs of global surgical procedures performed according to non-governmental approved tariffs in a selected hospital in Tehran. The results of this study can be utilized by hospital management for cost control and by insurance organizations. Materials and Methods: This retrospective and descriptive-analytical study was conducted cross-sectionally in a selected hospital in Tehran in 1400 (Iranian calendar year). It examines the global calculation of surgical costs for 864 global surgeries performed in this hospital compared with non-global calculations of these surgeries. The costs were investigated using Excel 2016 software for data analysis. Results: This study covered 16 common surgeries performed in the selected hospital out of a total of 100 surgeries approved in the global system in 1400. The findings showed that, based on the average cost of each global surgery, the global income was less than the average cost in all surgeries studied. The highest loss, in terms of both percentage and Rials, was related to the operation of repair of abdominal or primary incisional hernia, with an average difference of 19.22 percent and an average loss of 76,162,000 Rials per operation. The highest profitability, in terms of percentage and Rials, was related to diagnostic or therapeutic dilation and curettage, with an average difference of 8.96 percent and an average profit of 1,467,000 Rials per operation Conclusion: The results indicate that the global surgical procedure tariffs are not realistic and, due to weaknesses in the calculation method, result in losses for hospitals. Therefore, to prevent hospital losses and minimize flaws in the tariff calculation method, it is suggested that the currently announced global tariffs be reviewed. Consideration of factors such as patient age, presence or absence of concomitant diseases, disease severity, length of hospital stay, and method of discharge should be included in tariff calculations.
