fa ظرفیت‌ها و چالش‌های حوزه سلامت: یک مرور حیطه ای Capacities and Challenges of the Health Sector: A Scoping Review مقالات کامل full articles مروری review مقدمه: حوزه سلامت یکی از مهمترین بخش‌های کشور است که سیاست‌های آن وضعیت سلامت تمام مردم را تحت تاثیر قرار می‌دهد. شناسایی ظرفیت‌های حوزه سلامت یکی از مهمترین اقدامات برای تصمیم‌گیری درست در جهت اهداف نظام سلامت می‌باشد. مواد و روش‌ها: مطالعه حاضر یک پژوهش توصیفی – تحلیلی و از نوع کاربردی است که با استفاده از مرور حیطه‌ای انجام شد. برای انجام مرور از پروتکل شش مرحله‌ای آرسکی و اومالی استفاده شد. جامعه آماری مطالعه شامل اسناد بالادستی، رساله‌ها و پایان‌نامه‌ها، مقالات داخلی و خارجی در دسترس می‌باشند و برای جستجوی منابع، پایگاه‌های داده الکترونیکweb of science, scopus, magiran, pubmed و ایرانداک، ایرانمدکس sid.ir,ensani.ir, isc.gov.ir انتخاب شدند و این پایگاه‌ها با استفاده از کلیدواژه‌های مناسب و به‌صورت منظم  از سال 1385 تا آذر ماه 1402جستجو شدند، علاوه براین جستجوی دستی منابع مقالات بدست آمده نیزانجام شد. تحلیل داده‌ها با روش تحلیل محتوا انجام شد، در این روش داده‌ها از طریق فرایندهای نظاممند، کدبندی، تم‌سازی و طبقه‌بندی شدند.  از نرم افزار MAXQDA نسخه 2020 برای تجزیه و تحلیل داده ها استفاده شد. یافته‌ها: در فاصله زمانی جستجو شده 11 سند بالادستی و 53 مطالعه  دارای معیارهای ورود به مطالعه بودند که ظرفیت‌ها، چالش‌ها و راهکارهای حوزه سلامت را بررسی کرده بودند. ظرفیت‌ها شامل اسناد، بهبودهای قابل توجه در شاخص‌های سلامتی، توانمندی‌های علمی مثل نرخ رشد علمی سریع، نظام آموزش پزشکی توانمند، دانش فنی تولید واکسن جدید، شبیه سازی سلول‌های بنیادی و ساخت دارو بودند. چالشهای عمده شامل تولیت یا حاکمیت سلامت، تامین مالی، عدالت، تعارض منافع، گردشگری سلامت، همکاری بین بخشی، تاب‌آوری نظام سلامت و تعیین کننده‌هایی اجتماعی سلامت بودند. نتیجه‌گیری: نظام سلامت ایران با چالش‌های جدی روبه‌رو می‌باشد. با این حال کشور ما از ظرفیت‌های بالقوه و بالفعل قابل توجهی در این حوزه برخوردار است. توجه به ابعاد کلیدی همچون تولیت و حاکمیت کارآمد، تأمین مالی پایدار، تحقق عدالت در سلامت، مدیریت کارآمد تعارض منافع و تقویت همکاری‌های بین‌بخشی، می‌تواند زمینه‌ساز تحولات عمیق و پایدار در نظام سلامت کشور شود.   Introduction: The health sector is one of the most important sectors of the country, with its policies impacting the health of the entire population. Identifying the capacities of the health sector is a critical step in making informed decisions to achieve the goals of the health system. Materials and Methods: The current study is a descriptive-analytical and applied research conducted using a scoping review. Arsky and O'Malley's six-step protocol was used for the review. The study population included upstream documents, dissertations, theses, and accessible domestic and international articles. Electronic databases such as Web of Science, Scopus, Magiran, PubMed, Irandoc, IranMedex, SID.ir, Ensani.ir, and ISC.gov.ir were selected to search for resources. These databases were systematically searched from 2006 to December 2023 using appropriate keywords. Additionally, manual searches of references from retrieved articles were also conducted. Data analysis was perfomed with the method of content analysis. The data were coded, themed, and classified through systematic processes in this method. Data were analyzed using MAXQDA software version 2020. Results: During the search period, 11 upstream documents and 53 studies met the inclusion criteria, which examined the capacities, challenges, and solutions in the health sector. The identified capacities included strategic documents, significant improvements in health indicators, scientific capabilities such as a rapid growth in scientific output, a strong medical education system, technical expertise in developing new vaccines, stem cell simulation, and drug production. The major highlighted challenges were related to health governance, financial sustainability, equity, conflicts of interest, health tourism, intersectoral collaboration, health system resilience, and social determinants of health. Conclusion: Iran's health system is facing serious challenges. However, our country has considerable potential and actual capacities in this area. Optimal utilization of these capacities and leveraging past successful experiences can significantly enhance health indicators. In this regard, focusing on key aspects such as effective governance, sustainable financing, achieving equity in health, efficient conflict of interest management, and strengthening intersectoral collaborations can pave the way for profound and sustainable transformations in the country's health system.   ظرفیت‌های بالقوه و بالفعل,حاکمیت, تامین مالی, حوزه سلامت.
