fa بررسی تاثیر پزشکی از راه دور در طب نظامی: یک مطالعه مروری Effect of Telemedicine in Military Medicine: A Literature Review مقالات کامل full articles مروری review مقدمه: نیروهای نظامی به دلیل شرایط شغلی در معرض بیماری‌ها و خطرات خاصی هستند. این مطالعه مروری باهدف بررسی تاثیر پزشکی از راه دور در طب نظامی انجام شد. مواد و روش ها: یک مرور نظام‌مند در مطالعات، با جستجوی کلمات کلیدی مرتبط در عنوان، چکیده و کلمات کلیدی مرتبط در پایگاه داده‌های علمی معتبر PubMed، Embase، Scopus و Web of Science انجام گرفت. مقالات انگلیسی‌زبان در 30 دسامبر 2021  بدون محدودیت زمانی که مربوط به توسعه، نقش و ارزیابی تاثیر پزشکی از راه دور در طب نظامی بودند به این مطالعه وارد شدند. از سوی دیگر معیارهای خروج شامل مقالات مروری، چکیده‌های بدون متن کامل، نامه به سردبیر و عدم دسترسی به متن کامل مقالات بودند. معیارهای واجد شرایط بودن به‌طور مستقل توسط دو نویسنده غربالگری شدند. از چک‌لیستی یکسان برای استخراج داده‌هایی ازجمله رفرنس، سال انتشار مقاله، نام کشور، نام نویسنده اول، جمعیت و حجم نمونه، الزامات و رویکردهای پزشکی از راه دور و نتایج اصلی مطالعه استفاده شد. یافته‌ها: در ابتدا 250 مطالعه از جستجوی پایگاه‌های اطلاعاتی بازیابی و درنهایت 19 مطالعه وارد این بررسی شدند. 12 مورد از مطالعات در پادگان‌ها یا مناطق نظامی، 4 مورد در نیروی دریایی و 3 مورد دیگر در مناطق جنگی انجام‌شده است. پیامدهای موردبررسی در مطالعات شامل استفاده از پزشکی از راه دور در خدمات ارتوپدی (n=4) مراقبت‌های ویژه (n=3)، روانشناسی و روان‌پزشکی جراحی (n=5)، پایش فیزیولوژیکی و بیولوژِیکی سربازان (n=2)، سیستم تصمیم‌یار برای تلفات بیولوژیکی (n=1)، برنامه آموزش به پزشکان (n=4) ازجمله تروما بودند. رویکردهای ارتباطی مورداستفاده توسط پزشکی از راه دور شامل ویدئوکنفرانس آنلاین، ایمیل، تلفن، فناوری پوشیدنی، وب‌سایت و اپلیکیشن های مبتنی بر تلفن همراه بودند. اغلب رویکردها به‌صورت هم‌زمان و از طریق پلتفرم ویدئوکنفرانس آنلاین و وب‌سایت‌ها بودند. در تمامی مطالعات اثرات مثبت پزشکی از راه دور در پیامدهای موردبررسی برای ارائه مراقبت‌های بهداشتی در نیروهای نظامی گزارش کردند. ازجمله مهم‌ترین چالش‌های اجرای گسترده پزشکی از راه دور در محیط‌های عملیاتی، محدودیت‌های شبکه و نگرانی‌های امنیت سایبری بودند. ارتباطات مطمئن، پهنای باند بالا، تأخیر کم و ایمن برای تسهیل و استفاده بهتر از قابلیت‌های پزشکی از راه دور ضروری به نظر می‌رسد. بحث و نتیجه‌گیری: پزشکی از راه دور در طب نظامی پتانسیل بسیار بالایی در مراقبت از این نوع بیماران دارد. این به‌طور بالقوه نه‌تنها آمادگی عملیاتی مراقبین قابل‌استقرار را حفظ می‌کند، بلکه تجربه بیمار با به حداقل رساندن محرومیت‌ها از خدمات سلامت بهبود می‌بخشد. با توجه به محدودیت‌های امنیتی به‌عنوان مهم‌ترین چالش، باید طیف کاملی از گزینه‌های پشتیبانی پزشکی از راه دور را که شامل راه‌حل‌های پهنای باند بالا و ایمن توسعه داده و آموزش دهد. Introduction: Military forces are exposed to certain diseases and hazards due to occupational conditions. This review study was conducted to investigate the effect of telemedicine in military medicine. Methods and Materials: A systematic literature review of the studies was conducted by searching for relevant keywords in the title, abstract and related keywords of Embase, Web of Science, Scopus and PubMed in the authoritative scientific databases. English-language articles related to the development, role and effect of telemedicine in military medicine were included in the study on December 30 in 2021 without time limit. Exclusion criteria included review articles, full-text abstracts, letters to the editor, and lack of access to the full text of the articles. Eligibility criteria were independently screened by two authors. The same checklist was used to extract data such as reference, year of publication, country name, first author name, telemedicine approach, outcome, or training strategy. Results: Initially, 250 studies were retrieved from database searches and 19 studies were included in this study. Twelve studies were conducted in barracks or military areas. Four of them were performed in navy and three of them were done in war zone. Outcomes in studies including the use of telemedicine in orthopedic services (n= 4) intensive care (n= 3), psychology and surgical psychiatry (n= 5), physiological and biological monitoring of soldiers (n= 2), decision system for biological casualties (n= 1), physician training program (n= 4) including trauma were the predominant specialties that used telemedicine. Communication approaches used by telemedicine included online video conferencing, email, telephone, wearable technology, websites, and mobile applications. Most approaches were synchronous through the online video conferencing platform and websites. All studies reported the positive effects of telemedicine on the consequences of providing health care in the military. Among the major challenges facing the widespread implementation of telemedicine in operational environments were network constraints and cybersecurity concerns. Reliable communication, high bandwidth, low latency and security are essential to facilitate and make better use of telemedicine capabilities. Discussion and Conclusion: Telemedicine in military medicine has a very high potential in caring for this type of patient. This potentially not only maintains the operational readiness of reliable caregivers, but also improves the patient experience by minimizing deprivations of health care.
