Introduction: Hospitals must have an understanding of the future in order to encounter an uncertain future and succeed in a competitive environment. This allows them not only to meet patients' needs, but also to surpass current expectations. Scenario planning is an appropriate approach for highly uncertain environments. This study aimed to discover and identify future strategies for Iranian military hospitals using a future studies approach and scenario planning.
Materials and Methods: This purpose driven and survey-based study has been performed in exploratory area. The statistical population included various executive and managerial experts from military hospitals in Tehran. The sampling method was purposive and nonprobabilistic. The research was conducted in two phases: first, the mutual impacts of components were evaluated, and then strategies and policies were developed. Data were collected through interviews. The data were subsequently analyzed using MAXQDA and Scenario Wizard software.
Results: Six coherent scenarios were extracted from the mutual impact matrix. These scenarios included changes in hospital leaders’ perspectives on the importance of risk management and the need for comprehensive programs of crisis management to combat
emerging diseases. The scenarios also included the establishment of structurally and non-structurally resilient hospitals, the utilization of new technologies such as artificial intelligence, machine learning and the adoption of modern treatment models, the exchange of information and experiences during crises, the optimized management of supply chain and costs related to essential equipment and medicines.

Conclusion: The proposed scenarios play an important role in enhancing the preparedness and resilience of military hospitals. These scenarios enable rapid response exercises, coordination among different units, and optimal resource management by simulating real conditions of natural disasters, security threats, and human incidents.
